About Primary and Community Care
Les soins primaires et communautaires jouent un rôle central dans la détermination de la demande au sein du système de santé, les décisions cliniques relatives à la prescription, aux examens diagnostiques et au suivi ayant une incidence tant sur les résultats pour les patients que sur l’impact environnemental. Les soins prodigués dans ces contextes sont étroitement liés aux phénomènes de surdiagnostic, de surutilisation et de variations dans les pratiques, ce qui peut contribuer à une utilisation inutile des ressources et à une pression évitable sur le système. Parallèlement, les soins primaires et communautaires sont de plus en plus affectés par des pressions liées au climat, notamment la détérioration de la qualité de l’air, les phénomènes météorologiques extrêmes et les problèmes de santé liés au climat, ainsi que par des inégalités croissantes en matière d’accès et de résultats.
Cette section rassemble les principaux domaines de pratique où la durabilité environnementale recoupe la prestation quotidienne des soins primaires et communautaires, notamment la prescription, le diagnostic, les parcours de soins et les soutiens communautaires. Elle met en évidence les possibilités d’aligner des soins de haute qualité sur une utilisation plus efficace des ressources du système de santé et sur l’amélioration des résultats de santé de la population.
Les guides pratiques CASCADES et les ressources d’accompagnement fournissent des outils concrets et fondés sur des données probantes pour favoriser la durabilité tout au long des parcours de soins primaires et communautaires. Ces ressources portent sur la prescription, les modèles de soins et l’amélioration de la qualité à l’échelle du système, et sont conçues pour soutenir la mise en œuvre et la généralisation des initiatives de durabilité au sein des systèmes de santé.
